How do I schedule an appointment?
By calling toll-free 1-866-742-3226, option 2, you can schedule an appointment
for the clinic site most convenient for you.
What do I bring the day of my appointment?
- A photo ID (driver’s license, passport, etc.)
- Social Security card
- Copy of your birth certificate
- One month’s income statement (check stubs, W-2 form, bank statement)
If I am under 18, do my parents have to know?
We encourage as much parental involvement in this part of your healthcare as possible, but all of our services
remain confidential.
What if I don’t have insurance?
- You may apply for the Iowa Family Planning Network (IFPN) insurance program,
and your pregnancy prevention services (including your annual exam and birth control
method) will be covered.
- For those that do not qualify for the IFPN program, we do provide servcies on a sliding-fee
scale, based on your household income.
What types of servcies can I expect at my visit?
- Annual exams, including pap smears
