A community-led initiative serving families in Clayton and Fayette counties using a collective impact model focused on moving families from crisis to career through resource navigation, community partners, and a collaborative case management system.

How Thrive Iowa Supports Families

Navigation

Navigators connect participants to existing community resources, advocate on their behalf, and ensure service coordination among providers. Participants complete an assessment across 13 care areas and create an action plan, addressing crisis areas first. Navigators make referrals and assemble a support team to address each area of need. As stability grows, Navigators and participants work together to obtain and/or advance in employment with the assistance of the Community Alliance.

Community Alliance

The Community Alliance creates sustainable impact through a collective impact approach. It brings together public, private, and social sectors to broaden the scope of available resources and help ensure participants receive comprehensive care. Alliance meetings are held monthly to discuss topics that affect our communities. 

HopeHub

HopeHub is the collaborative case management system that connects organizations to create a cross-sector care team. HopeHub streamlines communication between care providers, maintains up-to-date community resources, enables easy referrals, and provides real-time data to track progress and community impact.

“Embrace Iowa helped our family be getting us a stove. The stove we had was hazardous. When we would cook on it, it would stink the whole house up. Also, it leaked underneath the stove. That stove was so small that my two year old son could see inside the pan without standing on anything.
